Comparative Experimental Study on the Effect of Truncal Vagotomy with Pyloroplasty, Selective Vagotomy with Pyloroplasty and Parietal Cell Vagotomy on Insulin Responded Gastric Acid Secretion


Abstract
Recently, more surgeon are believed- that, parietal cell vagotomy is one of best suitable operation for duodenal ulcer, because its low rate of complication, recurrence rate and low gastric acid secretion.
The purpose of this study is to prove common belief that parietal cell vagotomy is superior among various acidity reduction operation.
Free acidity were obtained in mongol dog which had truncal vagotomy with pyloroplasty, selective vagotomy with pyloroplasty and parietal cell vagotomy after administration of insulin.
Gastric juice analysis were carried out monthly for 3 times with adminstration of insulin. Each time gastric juice were obtained every 15 minute for 8 times and free acidity were measured.
The results were as follows;
1. The reduction rates of free acidities of 1 month, 2 month and 3 month after. parietal cell vagotomy (40. 74%, 70. 37o and 56.67% respectively) were greater than those after selective vagotomy with pyloroplasty (18. 52%, 41.2700 and 25.93061 respectively). And the. reduction rates of free acidities of truncal vagotomy with pyloroplasty were 70: 3700,¢¥ ¢¥47., 09% ¢¥ .and 83.07/61 respectively.
2. The reduction rates of insulin responded free acidities of 1 month, 2 month and 3 month after parietal cell vagotomy (76.74%, 64.16% and 40.53% respectively) were greater than those after truncal vagotomy with pyloroplasty (41.45%, 40.461/ and 39.631%/ respectively) and selective vagotomy with pyloroplasty (43.50, 53.7%and 23.491 respectively).
3. In parietal cell vagotomy and selective vagotomy, the values of 1 month and 2 month were negative for Hollander tests. But. in values of 3 month after both operations and truncal vagotomy, Hollander tests were late positive.
